Monthly Payments ( auto pay 1st of month CREDIT/DEBIT CARDS ONLY )
One Installment $640 (due with registration)
Club and Coaching Dues Total = $59 per month
Team Dues(We expect an additional 10-15 per month)
for leagues, tournaments and indoor practices
tournaments are optional (up to team parents)
Team Dues are invoiced on 8/1, 10/1 and 2/1
Annual Registration Fee $50
Jerseys approx $100 every 2-3 years
Jersey payment due with Aug 1 team dues
A 2% Fee will be added to Credit Card Transactions
By submitting this form I understand the following
My card will be charged for August Club Dues of $59 and for the Annual Registration Fee of $50
My card will be charged $59 on the first of each month thereafter from September - May
I understand Team Dues will be invoiced separately from Club Dues .
Team dues are invoiced on August 1st, October 1st and February 1st.
Team dues = total cost of leagues, tourneys and indoor field rental divided by total players
Jersey cost will be billed with August 1 team dues if applicable
I understand there will be a 2% fee on all credit/debit card transactions
I understand paying one installment only covers club/coaching dues and not team/jersey dues
